What is this program, how do i use it? where do i get it?

(picture in last link below)This program apparently checks your source for mem leaks or lag spikes or whatever, and i really need this! it looks kind of like jvisualvm, but i have no clue how to use that even. I am not sure how to use this. Could you maybe help me figure it out? Like when i open jvisualvm how do i add my server to the list to where i can see the memory being used in each class

when i run jconsole for example, it asks me for a hostname and port id. I tried putting in my ip and the port of my server but that didnt work so i am confused.
