We've been pushing out a lot of new updates lately. Kodai's most recent update is the barrows boss Scylla.
Scylla (level -666) - A custom barrows boss that switches between all the barrows brothers.
Upon opening a barrows chest (it is not relevant what your reward chance is) there is a chance of receiving this message as you get your loot.
I'm not going to cover all of the boss's mechanics. However, here is a clip of me fighting it where you can see the rocks falling and so on.

To address some of the things you said...
You actually don't need an e-mail to sing up. Just pick a username within the char limit and log in. This will create a forum account for you and set you off to play right away unless the name is taken. Though I can see how this can be explained better on our website.
I think you missed a pretty cool key feature for pvp, [the arena]. Though we want to improve PvP activity in general once we've pushed the Nightmare update coming next week.
I'm not sure if you played on our "dead" times, but I believe every day since you started recording we've reached a player count of 50. Might be the CC isn't always as active as people go in their own CC's to play COX. I know this was a bigger issue back when you were a regular player, but I don't think its nearly as bad now.
We're hoping to stand out more with updates like Scylla and more I cannot disclose at this time, but we're working on. Rather than trying to push only quality OSRS updates.
Every day I've logged in for the past week I've seen no less than high 30's or low 40s at worst with an exception after we've just updated and the following hours.
Sure chests and keys are a way to get people to donate, but it's also a nice way for f2p players to spice up their experience as you don't HAVE to donate to get these boxes. It's just easier to buy keys, I think that sounds a lot more fair to everyone rather than having items you can only donate for.
Been a follower of your channel for a while, hope to see you back once we've made the improvements we know we need! There is no better feedback than what you can thoroughly explain in a video so again, thanks for the feedback!

Nightmare media. Excuse the lack of interfaces as I'm not actually in the boss fight myself as I am recording this! If anyone were to enter the nightmare all interfaces would be there (health bars and all).
