Hq and Lq in the corner is pretty pointless, nice way of doing it thought. Name, Password and login = good. I'd rather have 4 than 3 buttons/boxes under the login button and maybe 4 of those funny lines down the boxes/buttons...
Conclusion 8/10 for creativity, 7/10 for outcome
