Add a exception in your update of a npc to a player.
So if you have a certain stage of a quest there is a npc on a specific spot. If you wish to, can also have a place holder npc that is invis. (Can't remember the id / ids for them)

How do Runescape make 'instanced' npcs? there are npcs that appear for one player, but not another because they haven't completed a certain quest
An example would be Zamorak mage wont appear in wilderness for the player unless they have completed the miniquest
