Hello, I've had incidents involving cheat clients spawning items not even in the server yet, anyone have any idea how to fix this? RS2HD!

"I force send packets to your server" or something like that, is what he says he does.

It only shows the item on the screen, but it's not really there

Does anyone remember AutoRune? AutoRune was a cheat client for the real runescape that was based on packet injection.
It allowed duping and easy high skills with the right scripts and enough knowledge about the protocol.
Jagex stopped this cheat client from working by introducing ISAACipher to their protocol.

This guy is probably not doing the same thing (some server side checks could probably stop it too, basically it is the result of poor programming), but it could be stopped anyways by using the ISAACipher.

No, it's there, what he did was force a blade with a hilt. Worked.

Have you checked his account to see if he really has these items?
